Job Description:
A company is looking for a Telephonic RN Nurse Care Manager.
Key Responsibilities
Manage a caseload of high-risk patients, coordinating their care
and addressing barriers
Prioritize patient needs and collaborate with a multidisciplinary
team to develop individualized care plans
Implement and evaluate comprehensive clinical care management plans
to reduce unplanned hospitalizations
Required Qualifications, Training, and Education
Active R.N. License
2 or more years of direct home health experience or care
management
Knowledge of Medicare regulations and home care and hospice
standards
Ability to perform extensive telephone assessments
Must be self-motivated, organized, and able to meet deadlines
