Job Description:

A company is looking for a Care Manager RN - Per Diem - National Remote.

Key Responsibilities

Coordinate patient-centered care and develop safe discharge plans
Facilitate communication among multidisciplinary healthcare teams and patients
Manage utilization of resources to improve patient flow and reduce readmissions

Required Qualifications

Associate's Degree (or higher) in Nursing
Current, unrestricted RN license in the state of residence
3+ years of experience in a hospital, acute care, or direct care setting
Intermediate proficiency in typing and navigating a Windows-based environment
Preferred: Bachelor of Science in Nursing (BSN) and case management experience
